And although many of these discoveries have a long way to go in clinical practice, they no longer belong to the category of conceptual developments, but are actually working devices, although not yet widely used in medical practice.

AbioCor Artificial Heart
In July 2001, a group of surgeons from Louisville (Kentucky) succeeded in implanting a new generation artificial heart into a patient. The device, called AbioCor, was implanted into a man who was suffering from heart failure. The artificial heart was developed by Abiomed, Inc. Although similar devices have been used before, the AbioCor is the most advanced of its kind.

In previous versions, the patient had to be attached to a huge console via tubes and wires that were implanted through his skin. This meant that the person remained confined to a bed. The AbioCor, on the other hand, exists completely autonomously inside the human body and does not need additional tubes or wiring that go outside.

Bioartificial liver
Dr. Kenneth Matsumura came up with the idea of creating a bio-artificial liver and decided to take a new approach to the question. He created a device that uses liver cells harvested from animals. Recently, the demand for medications to treat depression has increased: according to statistics, the number of patients receiving them in 2020 is 23% higher than in 2015. And due to the unstable economic situation in 2022, the demand for antidepressants is definitely not going to decrease.

But there are still a number of myths about antidepressants (even within the medical community). Let’s sort it out together.

Antidepressants are ineffective
Ineffectiveness is one of the most common myths about antidepressants. It is based on the fact that there are still gaps in the pathophysiology and mechanism of action of depression medications. Some researchers believe that the therapeutic activity of antidepressants is the result of side effects that increase patient expectations. A large-scale review of 15 studies involving more than 3,300 patients has dotted the i’s. Its results showed that participants without early adverse events who received citalopram or paroxetine had significantly greater relief of depression symptoms, as assessed by the Hamilton Depression Scale, than those who received placebo.

Impaired serotonin metabolism has nothing to do with the pathogenesis of depression, so selective serotonin reuptake inhibitors (SSRIs) are not pathogenic medications for its treatment
Some experts have long believed that the hypothesis about the effects of serotonin and its precursor 5-hydroxytryptophan on depression is nothing more than a tool for the pharmaceutical industry to promote a simplified biological model of this pathology and to stimulate the market for SSRIs. TCAs are practically never used in modern practice

Ethanol perverts the effects of drugs: it can increase or decrease their effectiveness, potentiate unwanted adverse reactions. At the same time, some drugs can aggravate the toxic effects of alcohol.

The interaction may be at the level of pharmacodynamics. For example, alcohol combined with a number of drugs can mutually increase the effect on CNS. Pharmacokinetic interaction may occur at the stage of absorption. For example, low-alcohol beverages may accelerate medication penetration into the blood, resulting in a sharp increase in concentrations of the latter can manifest as symptoms of overdose. This is possible when the interval between the intake of alcohol and the pill is small.

Alcohol can alter the pharmacokinetics of drugs by interfering with their metabolism, which occurs with the participation of the cytochrome P450 system.

When the concentration of ethanol in the blood reaches more than 1% (150 ml of vodka drunk), cytochrome system enzymes are involved in ethanol utilization. This means that the load on the enzymes increases and the elimination time of harmful substances, which the body considers to be both drugs and alcohol, increases. Alcoholic drink drunk by a non-drinker in a large dose inhibits functioning of all cytochrome enzymes. This means that the elimination period of the drug is longer, and its pharmacological effects and side effects are intensified. Regular consumption of alcohol in small and medium doses increases cytochrome P450 system activity. The opposite process occurs. The medication is metabolized faster and loses its effect.

In addition to the dose and the nature of alcohol consumption, other circumstances influence how alcohol behaves together with the medication in the body:

What kind of medication the patient is taking and its dose
strength of the drink, its type, and time of ingestion
overall condition of the patient, the severity of the illness
individual characteristics of body chemistry
gender, age, weight, comorbidities and others.

It is difficult to predict in advance the consequences of combining a drug with alcohol, since targeted studies involving people on this issue are not conducted, and too many factors play a role. However, certain combinations of drugs with alcohol are known to have clinically significant adverse effects that are definitely worth avoiding.

A number of medications inhibit alcohol dehydrogenase, which neutralizes the toxic breakdown product of alcohol, acetaldehyde. Accumulation of acetaldehyde in the body is expressed by symptoms of fever, shortness of breath, tachycardia, nausea, vomiting, general malaise, increased sweating, tremors, chest pain, heaviness in the head, a feeling of fear for life, and blood pressure may drop.

Alcohol and drugs used in cardiology

Ethanol is able to interact with many cardiology drugs, namely antianginal, antihypertensive, antiarrhythmic, cardiotonics. In almost all cases, the combination of alcohol and drugs of these pharmacological groups will lead to adverse effects, whether it is a change in pharmacological efficacy, the manifestation of adverse reactions or toxic effects.

When drinking alcohol during treatment with antidiabetic drugs, especially sulfonylurea derivatives (glibenclamide, gliclazide, glimepiride), the risk of hypoglycemia increases. In turn, sulfonylurea antidiabetic agents slow down alcohol metabolism. Sucrose-lowering effect of insulin can be both enhanced and weakened by alcohol. Patients taking metformin are at increased risk of lactoacidosis with chronic use of alcoholic beverages.

Alcohol and drugs affecting the CNS
Opioid analgesics (fentanyl, promedol, tramadol, buprenorphine, as well as codeine, a component of combined analgesics and anti-cough drugs), neuroleptics (aminazin etc.), tranquilizers (especially benzodiazepines), sleeping pills (barbiturates) have a depressing effect on the CNS. Alcohol increases sedation, may cause impairment of memory, coordination, loss of consciousness, and depression of the respiratory center.

First generation antiallergic drugs (dimedrol, tavegil, suprastin, ketotifen, diazolin, pipolfen) with alcohol exhibit sedative properties more pronounced. Headache, drowsiness, lethargy, lethargy, loss of consciousness may occur.

At the beginning of the twentieth century, medicine appeared to “stand on the shoulders” of these and other fine physicians. Since then, medical discoveries have followed one after another: insulin for diabetics, chemotherapy for cancer, hormone treatment for glandular dysfunction, antibiotics for tuberculosis, chloroquine for certain forms of malaria, dialysis for kidney disease, and open heart surgery and organ transplants. This list could go on and on.

But how close is medicine to achieving its goal now, at the beginning of the twenty-first century? Can it guarantee “an acceptable level of health for all people in the world”?

An unattainable goal?

Children climbing on the shoulders of their fellows find that they cannot reach all the apples: the juiciest apples still hang from the top of the tree. So, too, is medicine: it paces from one achievement to another, but the most coveted goal–good health for all–remains out of reach.

Although the European Commission reported in 1998 that “Europeans have never lived as long or in as good health as they do now,” it also found that “one in five people will die before they reach age 65. Approximately 40% of deaths will be from cancer, another 30% from cardiovascular disease… It is necessary to provide protection against new, health-threatening diseases”.

The German medical journal “Gesundheit” (“Gesundheit”) reported in November 1998 that infectious diseases like cholera and tuberculosis are of increasing concern. Why? Antibiotics “are no longer effective. More and more bacteria are no longer responding to at least one of the common drugs, and many are no longer responding to several at once.” Old diseases are returning and new ones are emerging, such as AIDS. The German pharmaceutical publication “Statistics ’97” reminds us: “For two-thirds of all known diseases (and there are about 20,000) no cure has yet been devised.

Can we hope for gene therapy?
More and more treatments keep being invented. For example, many believe that genetic engineering will be the key to solving many health problems. In the 1990s, the writings of American doctors such as Dr. French Anderson called genetic engineering “the newest field of medical research. The book “Gene Therapy” claimed that gene therapy could lead “medical science to new frontiers of innovation. This is especially true for the treatment of as yet incurable diseases” (“Heilen mit Genen”).

Scientists hope to eventually be able to treat congenital genetic diseases by injecting patients with corrective genes. It may even be possible to find a mechanism that causes malignant cancer cells to self-destruct. It is already possible to perform genetic diagnostics to determine a person’s predisposition to certain diseases. Some say that the next invention will be medications that take into account a patient’s genome. One famous researcher suggests that doctors will one day be able to “diagnose their patients’ diseases and inject them with the appropriate piece of DNA strand as a treatment.

But not everyone is convinced that gene therapy will be the magic wand for curing diseases in the future. In fact, according to surveys, some people don’t want their genetic characteristics analyzed at all. Many fear that gene therapy could prove to be a dangerous interference with nature.

Time will tell whether genetic engineering and other high-tech methods will live up to expectations. But don’t be too optimistic. The book “Clay Pedestal” describes a very familiar process: “A new treatment appears, it is lauded at medical meetings and in specialist journals. Its creators become famous among colleagues; the media sing the praises of progress. A period of euphoria – documented evidence in favor of a miracle cure – is followed by a period of gradual disappointment, which lasts from a few months to a few decades. Then a new remedy is found, which almost overnight replaces the previous one, which everyone is already convinced is ineffective and has stopped using it. Many of the remedies that doctors have stopped using because they are ineffective were not long ago considered common knowledge remedies.
